face-lift and optimal results what to take care of

Dear dr. Pataki,

I wish to accept your proposal for a face-lift at your Institution. I would like to ask some questions concerning the consent form in face lift, which you kindly provided me after the thorough examination and consultation.
I would like to give up smoking soon to follow your recommendations.

Here are the questions,

1/ page 2 - "dietary supplements and medications may delay and interfere with healing" - Which dietary supplements can I not take? I intend on living healthier before surgery, which would include vitamin supplements.

2/ page 3 - "Smokers have a greater risk of skin loss and wound healing" - Would you advise "Haemo-Laser Treatment in Emineo" or BEMER therapy?

3/ page 5 - "may still have the effects of smoking....if not enough time has passed" - The 6 month period we talked about would be "enough time"?..Is there any kind of examination determining "enough time"?

4/ FDA... is that an issue in Hungary?

5/ page 6- "adverse reactions ...herbal, and/or prescription medication " - I assume I will be advised about what medication I can take

6/"large dose vitamins - " - I take different supplements, vitamins of various sorts - Do you need to know which ones?

7/page 9 - I consent to financing the surgery, the use of my telephone and email address, use of photographs, disclosure of my health information - Who do I  give my consent to? Dr. Pataki? or Prémium Plasztikai Sebészet?

I would like to ask for a run through of what the surgery looks like. Dr. Pataki showed me videos of the operation, but I was not receptive at the time.

We are glad that you have chosen to undergo a face-lift at Premium Plastic Surgery in Hungary. Below we are answering your questions concerning the consent form in face lift and look forward to seeing you soon at out consultation once again.

We highly recommend to completely refrain from smoking in case you would like to be operated and wish to attain the best result.

1/ page 2 – “dietary supplements and medications may delay and interfere with healing” – Which dietary supplements can I not take? I intend on living healthier before surgery, which would include vitamin supplements.

Multiple factors ( even some dietary supplements) can lead to impaired wound healing. Rather lets see, what you are taking in order to decide if that supplement is beneficial for you. As indicated above, the nutritional needs of the wound are complex, suggesting that composite nutrition support would benefit both acute and chronic wound healing. In summary, proteins, carbohydrates, arginine, glutamine, polyunsaturated fatty acids, vitamin A, vitamin C, vitamin E, magnesium, copper, zinc, and iron play a significant role in wound healing, and their deficiencies affect wound healing.

2/ page 3 – “Smokers have a greater risk of skin loss and wound healing” – Would you advise “Haemo-Laser Treatment in Emineo” or BEMER therapy?

No. For the postoperative wound and scar treatment we suggest diode laser treatment in our institution. This therapy accelerates healing and scar maturing.

Approximately over 4000 substances in tobacco smoke have been identified, and some have been shown to have a negative impact on healing. It is well-known that smoking increases the risk of heart and vascular disease, chronic lung disease, stroke, and many kinds of cancers. Similarly, the negative effects of smoking on wound-healing outcomes have been known for a long time. Post-operatively, patients who smoke show a delay in wound healing and an increase in a variety of complications such as infection, wound rupture, wound and flap necrosis at face lifts, epidermolysis, and a decrease in the tensile strength of wounds. In the realm of plastic surgery, impaired healing in smokers has been noticed both in routine plastic surgery and in the placement of implants. Cosmetic outcomes also appear to be worse in smokers, and plastic and reconstructive surgeons are often reluctant to perform cosmetic surgeries on individuals who refuse to quit smoking.

Some employers, insurance companies, and other institutions sometimes test for nicotine — the active ingredient in tobacco products. There are a couple of ways to test for nicotine and cotinine, the product created after nicotine enters your body. (That’s because cotinine is more stable and lasts longer in your body. The only reason you’d have cotinine in your body is if you processed nicotine.) Qualitative testing simply looks for whether or not you have nicotine in your body. Quantitative testing actually measures the concentration of nicotine or cotinine in your body. It gives more information about your tobacco habits. It can tell whether you’re an active smoker or if you’ve recently quit. If you’re not a tobacco user, it can tell if you’ve been breathing in a lot of tobacco smoke or not.

Cotinine can show up in a blood or urine test. It can be tested by a blood test or a urine test.  The trust among doctor and patient is important. We usually do not ask patients to go for such test, it is enough for us if they proclaim that they have quit smoking or using other tobacco products.

4/ FDA… is that an issue in Hungary?

FDA is not an issue here in the sense it is depicted in the american consent form. However we have similar Administration Authorities for Drugs and Devices. In the case of your operation one of the really applies.

Of course we will be supplying you with the exact list of medicines.

Yes, we would like to know which ones you take 2 months before the operation. That refers to point 1/ page 2 .

You will be consenting to both the doctor and the company that is providing the background for this operation.

I would like to ask for a run through of what the surgery looks like. Dr. Pataki showed me videos of the operation, but I was not receptive at the time.

Dr. Pataki will provide a detailed description of the operation once again and go through the steps of the operation with you. We understand that the high resolution videos were very demanding at the first consultation. This second time we recommend using charts and drawings, with which the basics of the operation can be explained as well.
